Medical Escalation Ward
Portland ward is a 27 bedded mixed medical ward.
In order to maintain patient flow within the hospital medically optimised patients are transferred to Portland.

Long Days: 07:00 - 19:30

Different safeguarding processes.

Mental Capacity Assessments and DoLS.

Medications administration.

Differing therapy assessments such as Pt, OT, SALT etc.

Palliative care, fast track discharges and EOL patients.

Best interest meetings and working with social workers on a more frequent level.

We have a lot of elderly patients on the ward so elderly care and dementia care is heavily featured.

How to manage challenging behaviour from patients who are confused or frustrated.

Moving and Handling processes.

Working alongside the discharge coordinator and closely with discharge hub nurses.

Admission processes from other wards.

How MDT works and how handovers with all sorts of medical professionals benefit the ward.

Wound care - complex wounds, pressure ulcers etc.

Management of chronic conditions e.g. COPD, Diabetes, and working with the relevant nursing staff for these such as respiratory or diabetes, or home oxygen nurses.

This is a mixed specialty ward, we have patients from respiratory wards, stroke unit, cardiology wards, gastro and geriatric medicine.

Caring for patients with learning disabilities.

Enteral feeding.

IV administration of our poorly patients.

Working alongside Nurse In Charge; learning the auditing process and how staffing is allocated.

Discussions with relatives regarding patient care, social history and discharge plans.

Managing deteriorating patients when it arises.
